HPK uses CareMonkey to streamline our enrolment process, allowing us to securely store your enrolment details and reduce the amount of paperwork for you to complete.
You will shortly receive an email invitation from CareMonkey on our behalf, asking you to create a CareMonkey profile. This profile will provide HPK with essential enrolment information for your child.
In order to finalise your child's enrolment, you will need to complete this documentation by Friday 17 August 2018.
To finalise your enrolment via CareMonkey you will need;
- an Immunisation History Statement from the Australian Immunisation Register
- birth certificate/passport
- a credit or debit card to process your non-refundable deposit payment of $102.40 (including $2.40 surcharge)
