Thank you for accepting your offer for a place at Helen Paul Kindergarten in 2020. We look forward to welcoming you into the HPK community.
HPK uses CareMonkey to streamline our enrolment process, allowing us to securely store your enrolment details and reduce the amount of paperwork for you to complete.
By 14 June 2019, you will receive an email invitation from CareMonkey on our behalf, asking you to create a CareMonkey profile. This profile will provide HPK with essential enrolment information for your child. 

In order to finalise your child's enrolment, you will need to complete this documentation by no later than 21 June 2019. If we have not received your completed CareMonkey forms by this date your child's place at Helen Paul Kindergarten may be forfeited.

To finalise your enrolment via CareMonkey you will need;

  • an Immunisation History Statement from the Australian Immunisation Register

  • birth certificate/passport

  • a credit or debit card to process your non-refundable deposit payment of $153.54 (including $3.54 surcharge)

  • valid Concession card (if applicable)

Please note, due to the ‘No Jab, No Play’ legislation Helen Paul Kindergarten must have a copy of your child’s Immunisation History Statement from the Australian Immunisation Register (AIR) at the time of enrolment.

We are unable to process your child’s enrolment without this document. Please note that this is the only acceptable form of evidence of your child's immunisation status. Immunisation records from overseas or letters from a healthcare provider are not an acceptable substitute.

Your child’s Immunisation History Statement can be found using your Medicare online account through myGov. You can also use the Express Plus Medicare mobile app.

Alternatively, you can also get your child’s Immunisation History Statement by:

  • going to a service centre, or

  • calling the AIR enquiries line on 1800 653 809 (Mon - Fri, 8am - 5pm)

Please note, it can take up to 10 business days to receive your child’s Immunisation History Statement in the mail.
